Prevention and chronic conditions: a model in Liguria combines health and sustainability

An ageing population is one of the main challenges facing healthcare systems in developed countries. The rise in chronic conditions is leading to an increasing demand for care, making it essential to develop integrated strategies for prevention, early diagnosis and the management of chronic conditions, not only to improve citizens’ health but also to ensure the sustainability of the healthcare system.
The model
With this aim in mind, between 2017 and 2019, ATS Liguria Area 3 implemented a multidisciplinary programme for health promotion and the prevention of chronic diseases, comprising lifestyle interventions, cardiovascular rehabilitation, metabolic screening, innovative models for diabetes management, telemedicine for complex patients and care pathways tailored to frail older people. implemented a multidisciplinary programme for health promotion and the prevention of chronic diseases, comprising lifestyle interventions, cardiovascular rehabilitation, metabolic screening, innovative models for diabetes management, telemedicine for complex patients and care pathways tailored to frail older people.
To assess its effects, the Genesis project (GENoa Epidemiology for Sustainable Health Improvement Study) was developed, based on an analysis of administrative data relating to over 700,000 residents in the Genoa metropolitan area, carried out by CliCon S.r.l. Società Benefit.
Focus on 3 conditions
The study analysed three chronic conditions with a significant clinical, social and economic impact that are closely linked to ageing: heart failure, chronic kidney disease and chronic liver disease.
The results show that, although the average age of the population rose from 48.8 to 50.9 years between 2011 and 2023, the increase in the prevalence of these conditions slowed in the years following the launch of the programme. This is a particularly significant finding in one of Europe’s oldest regions, where population ageing is generally associated with a progressive increase in the burden of chronic diseases.
